Lost entry form from DIAN and am leaving.

I am a US Citizen with a Colombian Visa and will be traveling to the US later this week. I last entered Colombia over a year ago (Sep. 2007) as a tourist and later got the visa. I was looking through my paperwork last night and it seems I can't find the form 530 from DIAN that was given to me to make my declarations at customs. Do I need it? What happens if I can't find it. I know I won't find it. However, I did find an older one that was given to me when I first entered in July of 2007. I'm flying American if it makes a difference.

I don´t think they are giving those out anymore....besides...it is a non-issue because you have a Colombian visa now and live here. They will treat you like a Colombian and charge to the extra departure tax.
